PROGRAM SPECIALIST II

Program Specialist II will provide care coordination to participants in the Shelter and will work in partnership with Shelter staff and community partners to ensure service needs are being met and clients are working toward their goal of safe housing. The Program Specialist I will engage in the process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual’s and family’s comprehensive needs through communication and available resources to promote patient safety, quality of care, and cost-effective outcomes.  The Program Specialist must be familiar with the various services in the community and make appropriate referrals so that clients can acquire the resources they need to establish and maintain their own households. The Program Specialist will maintain a focus on client self-determination and provision of client-centered and culturally relevant care, while ensuring quality of care, safe transitions, and timely access to services. This position will fulfill their duties Monday - Friday, working 7am-4pm M, T, TH, F and 8am-5pm on Wednesday. 

Full time position: 40 hours/week

Salary: $17.60 per hour

PROGRAM SPECIALIST I - Weekend

The Program Aide Position will be responsible for creating a welcoming environment for Shelter Residents. Staff will build relationships with community partners in person and over the phone and respond to requests and concerns in a professional and responsive manner.  This position will ensure the smooth operation of the reception area and the office in general, while completing administrative tasks identified by the Program Director, and will be responsible for general administrative duties listed below. This position will fulfil their duties Monday-Friday 4pm-12am.

Full time position: 40 hours/week

Salary: $15 per hour
